高龄老年冠心病患者冠状动脉病变的造影特点

摘    要:目的 观察高龄老年冠心病患者发病的危险因素与冠状动脉病变的影像学特点。方法 将 2 0 0 0年以来经选择性冠状动脉造影确诊为冠心病的 6 0岁以上老年患者共 15 2例 ,分为两组 ,高龄老年组 (≥ 75岁 )与普通老年组 (<75岁 )。比较两组冠状动脉病变部位、狭窄程度、侧支循环、室壁运动情况 ,并进行冠心病危险因素比较。结果 高龄老年组冠状动脉病变以左主干病变为主 ,而且侧支循环较丰富 ,与普通老年组相比 ,有显著性差异。多元相关分析显示高血压病程、糖尿病病程及心肌梗死病程和与冠状动脉病变严重程度呈明显正相关。结论 高龄老年组患者以左主干病变为主 ,且侧支循环较丰富。高血压病程、糖尿病病程及心肌梗死病程对冠状动脉病变程度具有重要影响

Abstract:Objective To evaluate the risk factors and coronary angiographic characteristics of the elderly patients with CHD.Methods A total of 152 patients were divided into two groups according to the dividing point of age of 75 years. Their risk factors and coronary angiographic characteristics were compared.Results In the group older than 75 years, the lesions were mainly found in the left main trunk and the collateral circulation was more abundant than the group under 75 years of age. A significant positive correlation between the severity of coronary artery lesions and the course of myocardial infarction, hypertension and diabetes mellitus was found from the multiple regression analysis.Conclusions In CHD patients of advanced age, the lesions are mainly found in the left main trunk, the collateral circulation is abundant, and the course of myocardial infarction, hypertension and diabetes mellitus have important correlation with the severity of coronary artery lesions.
