急性心肌梗死动物模型心电图波形变化规律与室性心律失常的相关性研究

摘    要:目的:分析急性心肌梗死(AMI)动物模型心电图波形PR间期、QRS时限、QT间期和QTc间期变化规律与室性心律失常的关系。方法:选择清洁级Wistar近交系30只大鼠,依照体重量将其编号后采用随机数表法分为假手术组(10只)和模型组(20只);模型组大鼠开胸腔建立AMI动物模型,假手术组不进行缝合结扎,其余方法同模型组;造模28周时检测两组大鼠心功能,并行心电图检查;采用logistics回归模型分析两组大鼠心电图PR间期、QRS时限、QT间期和QTc间期变化规律与室性心律失常的相关性。结果:模型组大鼠左室收缩末径(LVIDd)、左室舒张末径(LVIDs)均明显高于假手术组,射血分数(EF)、左室短轴缩短率(FS)均明显低于假手术组,且差异存统计学意义(t=16.865,t=12.954,t=10.631,t=16.341;P<0.05);模型组大鼠心电图PR间期、QRS时限、QT间期及QTc间期均明显高于假手术组,且差异存统计学意义(t=3.519,t=3.715,t=5.669,t=7.303;P<0.05);模型组中心律失常大鼠心电图PR间期、QRS时限、QT间期及QTc间期均明显高于心率正常大鼠,且差异存统计学意义(t=3.335,t=3.335,t=3.762,t=7.691;P<0.05);心电图PR间期、QRS时限、QT间期及QTc间期延长均是影响室性心律失常的独立性危险因素,且差异存统计学意义(OR=1.775,OR=1.984,OR=2.016,OR=1.921;P<0.05)。结论:AMI动物模型心电图PR间期、QRS时限、QT间期和QTc间期延长是影响室性心律失常的独立性危险因素。

Study on the relationship between the changes of ECG waveform and ventricular arrhythmia in animal model of acute myocardial infarction

Abstract:Objective:To analyse the relationship between changes in PR,QRS,QT,QTc interval and ventricular arrhythmia of AMI animal models.Methods:The Wistar rats were randomly divided into sham operation group(n=10)and model group(n=20)according to body weight number.The rats of model group received thoracotomy and be established to AMI animal model.The rats of sham operation group also received thoracotomy but didn’t be sutured and ligated.Heart function and electrocardiogram were measured at 28 weeks after modeling on the two groups of rats;By logistic regression model,the relationship between changes in PR,QRS,QT,QTc interval and ventricular arrhythmia was analysed.Results:The LVIDd and LVIDs of model group rats were significantly higher than those of sham operation group,and the EF and FS of model group rats were significantly lower than those in the sham operation group,and the differences were statistically significant(t=16.865,t=12.954,t=10.631,t=16.341,P<0.05);The interval and QTc interval of model group rats were significantly higher than those of sham operation group,and the differences were statistically significant(t=3.519,t=3.715,t=5.669,t=7.303,P<0.05).The PR interval,QRS time limit,QT interval,and QTc interval of arrhythmia group were significantly higher than the rats with normal heart rate,and the differences were statistically significant(t=3.335,t=3.335,t=3.762,t=7.691,P<0.05);PR interval,QRS interval,QT interval,and QTc interval extension were independent threat factors affecting ventricular arrhythmia,and the differences were statistically significant(OR=1.775,OR=1.984,OR=2.016,OR=1.921,P<0.05).Conclusion:The prolongation of the PR interval,QRS interval,QT interval,and QTc interval in AMI animal model rats are independent threat factors affecting ventricular arrhythmias.
