妊娠高血压综合征35例剖宫产手术的麻醉处理

目的总结妊娠高血压综合征(妊高征)行刮宫产术的麻醉处理过程。方法对35例妊高征患者在围术期采取一系列救治措施：①有效防治子痫发作；②麻醉诱导要求占时短，循环干扰少，能有效抑制有害应激反应和消除病人的精神紧张和不安，消除因疼痛而引起血压继续升高；③合理降低高血压，改善血流动力学，避免术中发生高血压危象；④改善肾脏及子宫血流灌注；⑤避免或慎用对新生儿呼吸有抑制作用的麻醉药。结果全组病例经麻醉处理后，循环稳定，均无发生任何并发症，经术后进一步积极支持治疗后全部康复。结论术前全面了解病情，充分完善术前准备，选择合理的麻醉方法，围术期对患者的合并症进行积极治疗，采取止惊、降压、强心、扩容等综合措施，是提高妊高征患者麻醉成功的重要因素。

Anesthetic Management of Cesarean Section in 35 Cases Patients with Pregnancy Induced Hypertension Syndrome

Objective To summarize the anesthetic management experience on cesarean section patients with pregnancy induced hypertension syndrome. Methods A series of therapeutic measures were done for 35 cases with pregnancy induced hypertension syndrome. These measures did prevent eclampsia effectively. Shorten the time for inducing anesthesia, reduce the interference, depress the bad stress reaction, eliminate the mental stress and uneasiness. Reduce the hypertension, improve hemodynamics and avoid the occurrence of hypertensive crisis. Improve the perfusion of kidney and uterus. Save the use of the narcotic to a great degree, that could inhibit the respiration of newborns. Results After anesthetic management for all patients, the blood circulation was stable and with no complications. All patients recovered after the supporting treatment in post operation. Conclusions Knowing the patients' conditions helps in preparing the preoperative, choosing reasonable anesthetic method, treating the complications of patients in perioperative period and taking comprehensive measures (such as anti convulsion, blood pressure lowering, cardiac and fluid expansion), which are important factor in successful anesthesia of patients with pregnancy induced hypertension syndrome.
