髂内动脉置管化疗对中下段直肠癌术后远期效果影响

目的探讨髂内动脉置管区域化疗对预防中下段直肠癌术后局部复发和肝转移的价值.方法 124例直肠癌术前随机分成两组,髂内动脉插管化疗组(治疗组)61例,外周静脉化疗组(对照组)63例,对两组疗效进行分析.结果治疗组1,3,5年生存率分别为100.0%、82.0%和52.5%;局部复发率和肝转移率分别为11.5%和18.0%.对照组1,3,5年生存率分别为100.0%、65.1%和31.7%;局部复发率和肝转移率分别为28.6%和23.8%.两组比较,3、5年生存率和局部复发率差异有显著性,肝转移率差异无显著性.结论髂内动脉置管化疗在预防中下段直肠癌术后局部复发和提高3、5年生存率方面优于外周静脉化疗.

Influence of internal iliac artery chemotherapy on long-term postoperative efficacy in treating middle-lower rectal carcinoma

Objective To evaluate the ef ficacy of internal iliac artery chemothe rapy in preventing local recurrence and hepatic metastasis after radical operati on of middle-lower rectal carcinoma.Methods 124 patients with middle-lower rec tal carcinoma undergoing radical resection were divided into 2 groups:internal i liac artery chemotherapy group(treatment group)61 cases,and peripheral venous ch emotherapy group (control group)63 cases,and the therapeutic results were analyz ed.Results In the in ternal iliac artery chemotherapy group,the 1-,3-and 5-year survival rates,loc al recurrence rate and hepatic metastasis rate were 100.0%,82.0%,52.5%,11.5% and 18 .0% respec tively,while in the peripheral venous chemotherapy group,the 1-,3-,and 5-year survival ra tes,local recurrence rate and hepatic metastasis rate were 100.0%,65.1%,31.7%,28 .6%and 23.8% respectively.Comparison between the two groups showed that the diff erences in 3-,5-year survival r ates and local recurrence rate were significant,while the hepatic metastasis r ate had no significant difference.Conclusion The efficacy of internal iliac artery chemothe rapy in preventing postoperative local recurrence and increasing 3-,5-year sur vival rates is superior to peripheral venous chemotherapy in patients with middle-low er rectal carcinoma.
