舌下神经鞘瘤的CT和MRI表现及其病理基础(附4例报告)

目的 研究舌下神经鞘瘤的CT、MRI特征性表现。方法 回顾分析了4例经手术病理证实的舌下神经鞘瘤CT和MRI表现。结果 从病变部位看，右侧3例，左侧1例；颅内段2例，颅内与颅外段同时受累2例。CT表现：舌下神经孔扩大和骨质破坏，并见软组织块影2例。MRI表现：舌下神经径路上出现圆形、椭圆形的软组织块影4例，T1WI为等信号，T2WI为等信号和高信号；有明显的强化。结论 舌下神经径路上出现软组织块影是其特点；MRI对舌下神经鞘瘤的诊断优于CT，特别是MRI增强扫描。

CT and MRI findings of hypoglossal schwannoma and their pathologic basis:a report of 4 cases

Objective To investigate CT and MRI findings of hypoglossal schwannoma (HS) and their relation to pathology.Methods CT and MRI findings in 4 patients with surgically and pathologically proven HS were analyzed retrospectively.Results For the distribution of HS,on the one hand,the lesions located in the right and the left were found in 3 cases and 1 case,respectively,on the other hand,the invasions of intracranial segment were found in 2 cases whereas the both invasions of intra- and extracranial segments were also found in 2 cases.CT revealed the enlargement and the destruction of hypoglossal nerve canal and soft tissue mass in 2 cases.MRI revealed the round and oval soft lissue mass along hypoglossal nerve pathway in all cases,which were of isointense on T1WI,isointene and hyperintense on T2WI as well as presented marked enhancement after enhanced scan.Conclusion The soft tissue mass shadows occurred along hypoglossal nereve pathway are characteristic findings of HS.MRI,specifically,MR enhanced scan is superior to CT for detecting HS.
