摘    要:目的 分析前列腺癌(PCa)磁共振波谱成像特征与细胞密度及增殖细胞核抗原(PCNA)表达的相关性,探讨1H-MRSI评估PCa细胞增殖状态的价值.方法 资料齐全的PCa患者38例,采用1.5T高场强超导MR成像仪,腹部相控阵线圈,3D-1H-MRSI 采用CSI-3D-Prostate序列扫描.根据常规HE染色和免疫组化观察PCa细胞密度及PCNA表达情况.结果 PCa和正常外周带的(Cho+Cr)/Cit比值分别为3.98±0.12 和0.38±0.09,差异有统计学意义(P<0.05),前列腺癌(Cho+Cr)/Cit比值与细胞密度、PCNA及Gleason分级存在正相关(r=0.495、0.582、0.431,P<0.05).结论 前列腺癌(Cho+Cr)/Cit比值高低与细胞增殖状态有关,1H-MRSI能对前列腺癌细胞增殖状况进行评估.

Abstract:Objective To study the relationship between MRI/MRSI and cell proliferation in prostate cancer (PCa). Methods A total of 38 patients with histologically proven PCa were included. The examination was performed in the supine position with a 1.5T superconductive magnet scanner (Siemens Sonata) with a pelvic phased array multi-coil. Three-dimensional proton MR spectroscopic imaging (3D-1H-MRSI) was acquired using CSI-3D-Prostate sequence. The choline plus creatine-to-citrate ratios ( /Cit) of PCa and peripheral zone (PZ) were calculated by spectroscopy software. The cellularity of PCa was recorded according to hematoxylin and eosin (HE) staining. The PCNA was detected using immunohistochemical technique. Results The (Cho+Cr)/Cit of PCa and PZ were 3.98±0.12 and 0.38±0.09 (P<0.05), respectively. (Cho+Cr)/Cit ratios were correlated positively with cellularity, PCNA and Gleason grade (r=0.495, 0.582, 0.431, P<0.05) of PCa. Conclusion There is positive correlation between MRI/MRSI and proliferation of PCa. MRI/MRSI can help to eveluate variables of proliferation of PCa.
