摘    要:动脉粥样硬化(atherosclerosis,AS)是一种慢性炎症性疾病,其特征是脂质、免疫细胞和纤维成分在动脉壁中积聚。目前临床诊断技术缺乏准确性和特异性,药物很难直接靶向斑块实现有效治疗。因此,如何对AS易损斑块进行准确诊断和有效干预、实现治疗药物的精准递送,成为目前临床迫切需要解决的问题。近年来,随着纳米科技和纳米材料的快速发展,纳米技术在监测易损斑块形成、提高斑块内药物浓度等方面表现出了独特优势,本文按照纳米材料类型分别对其进行综述,以期为纳米技术在AS疾病精准诊疗领域的研究和应用提供有益借鉴。

Research Progress of Nanotechnology in Diagnosis and Treatment of Atherosclerosis

Abstract:Objective To investigate clinical evaluation and potential mechanism of Shenfu Kang Capsule (SKC) in treatment to chronic nephritis (CN) based on integration meta analysis and network pharmacology research strategy. Methods The Chinese and English literature of the randomized controlled trials (RCTs) about SKC were comprehensively searched. The quantitative literature was determined based on the criteria of inclusion and exclusion. The meta analysis of the included data was carried out by the review manager. The chemical components, the target of action, disease targets, common targets, and other related information of SKC were screened based on network pharmacology. The key pathway was annotated. The potential mechanism of SKC intervention in CN was explored. Results A total of 11 RCTs were included, which were involving 938 patients. Meta analysis showed that the effective rate, blood lipid, lymphocyte subsets, fibrinolytic system, and renal function indexes in the therapy group were superior to those in the control group (P < 0.05). A total of 33 chemical components and 97 potential targets were obtained through the online database, which was related to AGE/RAGE pathway, lipid and atherosclerosis, TNF signaling pathway, IL-17 signaling pathway, and other molecular processes. Conclusion Current evidence shows that SKC can enhance the clinical efficacy of CN based on conventional western medicine treatment, and has certain advantages in regulating immunity, reducing lipid and anti-fibrosis. However, limited by the level of evidence, large-scale and multi-center RCTs are still needed for further research. SKC intervention in CN is an integration effect of multi-component, multi-target, multi-links integration effect, mainly through the regulation of autoimmunity, oxidative stress, inflammatory response, lipid metabolism, and other ways to achieve therapeutic purposes. This study is a research attempt based on the combination of Meta analysis and network pharmacology, and also provides a reference for clinical evaluation and mechanism research of other drugs.
