岛叶低级别胶质瘤的显微外科治疗

目的 探讨显微手术切除岛叶低级别胶质瘤的方法和效果。方法 对30例岛叶低级别胶质瘤病人的临床资料进行回顾性分析。结果 肿瘤全切除12例，近全切除14例，部分切除4例，无手术死亡。术后早期偏瘫6例、失语3例、偏瘫和失语3例，经术后3个月随访，仍有2例轻偏瘫、1例失语。结论 经外侧裂入路应用显微外科技术是治疗岛叶低级别胶质瘤的较理想方法。

Microsurgical treatment of insular low grade gliomas

Objective To explore the operative method and effect of insular low grade gliomas with microsurgical treatment. Methods A series of 30 patients with insular low grade gliomas were analysed retrospectively. Results According to early postoperative MRI imaging, complete tumor removal was achieved in 12 patients, nearly complete in 14 patients, and incomplete resection in 4 patients. There was no operative mortality. Twelve patients experienced immediate postoperative morbidity, including hemiplegia in 6 patients, aphasia in 3 patients, and hemiplegia and aphasia in 3 patients . After follow up review of 3 months three of 30 patients suffered permanent deficits, including mild hemiplegia in 2 patients and mild aphasia in 1 patient. Conclusions Microsurgery via transsylvian approach is a safe and effective treatment for insular low grade gliomas.
