内源性硫化氢与早产关系的探讨

摘    要:目的 探讨内源性硫化氢与早产的关系。方法 选取无妊娠并发症和内外科合并症、行剖宫产的产妇,分为A、B两组,A组为未使用任何药物已临产的早产者,B组为未临产的足月妊娠者,在麻醉前抽取静脉血5ml,分离出血清后用硫化氢ELISA试剂盒测定血清中硫化氢含量。胎儿娩出后、缩宫素使用前于子宫切口上缘中线处横行剪取少许子宫平滑肌条,进行肌条等长张力测定实验。结果 硫化氢对足月未临产子宫平滑肌的收缩没有明显抑制作用,对早产临产子宫平滑肌的收缩具有抑制作用,且呈剂量依赖性。早产组孕妇的血清硫化氢浓度明显高于正常组孕妇。结论 内源性硫化氢可明显抑制早产组子宫平滑肌的收缩,且呈剂量依赖性,早产组孕妇的血清硫化氢浓度明显高于正常妊娠组,推测是为了起到舒张子宫平滑肌以维持继续妊娠的目的。

Abstract:ObjectiveTo investigate the relationship between endogenous hydrogen sulfide(H2S) and premature labor. Methods Myometrium specimens were obtained by biopsy from pregnant women undergoing cesarean section, including 30 cases of preterm pregnany in labor(group A) and 30 cases of term pregnancy not in labor(group B). Venous blood samples were taken before anesthesia to determine H2S content in serum. Isometric tension of muscle strips, which were taken after delivery and before oxytocin administration, was measured. Results H2S had no significant relaxation effect in term non-labor uterine smooth muscle, but it caused a dose-dependent suppression of spontaneous contractions in premature term uterine smooth muscle. The serum concentration of H2S in premature pregnant women was significantly higher than that in normal group. Conclusion Serum H2S levels in premature pregnant women are high, which can suppress contraction of uterine smooth muscle to maintain pregnancy.
