摘    要:目的 探讨经会阴前列腺穿刺针数的选择与前列腺体积的关系。方法 回顾性分析379例临床怀疑前列腺癌患者穿刺活检的资料,以病理结果为金标准,分析不同体积,不同穿刺针数之间的穿刺阳性率。结果相同体积下,不同穿刺针数间的穿刺阳性率差异没有统计学意义;相同穿刺针数下,8针穿刺方案中不同体积组之间的穿刺阳性率差异没有统计学意义,而10针和12针穿刺方案中,差异有统计学意义,前列腺体积越小,穿刺阳性率越高。结论 前列腺体积是经会阴前列腺穿刺最佳针数选择中的重要相关因素,在穿刺针数的选择中应充分考虑前列腺体积的影响。本研究结果表明不同体积间应采取最优的8针穿刺方案,并对可疑结节行额外1~2针穿刺。

Abstract:Objective To investigate the relationship of optimal number of cores with prostate volume in transperineal prostate biopsy. Methods The biopsy data and results of 379 patients with clinically suspected prostate cancer were retrospectively reviewed. Taking positive rate of pathologic results as gold standard, the optimal number of cores for different gland volume was analyzed. Results The positive rate of biopsy was ranging from 47.22% to 11.43%. There no significant difference in positive rate among different transperineal biopsy numbers for the same gland volume; and also no significant difference in positive rate among different gland volumes by 8-core scheme with the same biopsy number. While there was statistically significant by 10-core and 12-core schemes for the different gland volumes, the positive rate was negatively correlated with the gland volume. ConclusionThe prostate volume is a related factor for choosing the best scheme of transperineal biopsy. The results show that the optimal scheme of 8-core should be taken for different volume prostate glands, and 1-2 extra transperineal biopsy should be added for suspicious nodules.
