肠内营养粉安素对老年患者糖、脂代谢及营养状况的影响

摘    要:目的 探讨安素对老年患者糖、脂代谢及营养情况的影响,为肠内营养在老年患者中的合理使用及效果评估,寻找有效的临床检测指标。方法 150例老年患者,根据营养及进食情况分为3组。以正常进食者作为对组照;进食减少者,予口服安素补充营养,为加用安素组;因疾病不能进食者,予安素经鼻饲提供肠内营养,为单用安素组。所有受试者及对照组每日提供的总热量为125.52kJ/kg。2周后留取血标本,检测餐后及空腹血糖、糖化血红蛋白、脂代谢指标、肝肾功能、高敏C反应蛋白。结果 与对照组相比,应用安素的两组老年患者血浆白蛋白水平均降低,球蛋白水平及高敏C反应蛋白水平均增高,而血浆前白蛋白水平无统计学差异(P=0.078,P=0.374)。结论 血浆前白蛋白是较好评估老年患者肠内营养效果的临床指标;肠内营养粉安素,可使老年患者的糖脂代谢、营养状况不同程度获益。

Abstract:Objective To investigate the efficacy of Ensure for enteral nutrition in aged patients. Methods One hundred and fifty aged patients were divided into 3 groups: in control group patients had normal oral diet, in study group 1 patients had normal diet with supplemental Ensure and in study group 2 patients were fed with Ensure via nasogastric tube. The total calories were around 125.52kJ/kg. The fasting blood glucose, postprandial blood glucose, glycated hemoglobin, serum lipids, hepatic and renal function and high-sensitivity C-reactive protein(hs-CRP) were measured two weeks after nutritional support. Statistical analysis was performed with SPSS 17.0. ResultsThe level of serum albumin in both two study groups was lower, and the levels of serum globulin and hs-CRP were higher than those in control group, while there were no significant differences in levels of serum prealbumin(P=0.089,P=0.347). There were no significant differences in levels of serum creatinine, blood urea nitrogen, microalbuminuria, urinary retinol binding protein, urinary transferrin, urinary beta-2 microglobulin among three groups. Conclusion Aged patients may benefit from the enteral nutrition Ensure in glucose, lipid metabolism and nutritional status. It is recommended that the aged patients with reduced normal oral diet due to the illness be supported by enteral nutrition Ensure.
