髁状突脱位骨折坚固内固定术25例疗效观察

 目的评价髁状突脱位骨折开放复位并行坚固内固定(RIF)的长期治疗效果.方法25名患者术后接受了平均4.5 a的随访调查.根据( )关系、颞下颌关节功能恢复情况和术后X线片评价术后效果.结果临床术后效果令人满意.放射学上,尽管骨折段经准确对位,但32%患者髁状突骨质与升支高度仍有改变.一般完全暴露和无血供的髁状突较部分保留血供的髁状突术后改变大.结论坚固内固定对髁状突脱位骨折是有满意疗效的.

Rigid internal fixation in treatment of dislocated condylar process fractures - a clinical analysis

Objective To evaluate the long-term results of rigid internal fixation for treatment of dislocated fractures of condylar process. Methods Twenty-five patients with fracture of condylar process who underwent open reduction and rigid internal fixation were followed up for an average of 4.5 years and evaluated on the basis of occlusion, temporomandibular joint (TMJ) function, and as radiological changes in condylar process. Results On clinal examination, the results were satisfactory in both occlusion and TMJ function. Radiologically, 32% of the patients showed a normal configuation of condylar process,and in the other cases, normal function was established even though there were condylar changes more or less. Despite alignment of the fractured segments was achieved in operation, a slight medial deviation of the condylar process were found in posterioanterior radiograph. The changes were more prominent in fully exposed and devascularized condylar processes than in partially devascularized condylar processes. Conclusion Open reduction and rigid internal fixation achieve satifactory result for the treatment of dis- located condylar process fracture.
