岛叶与杏仁核在癫痫中关系的实验研究

摘    要:目的:通过对普鲁卡因注射入癫痫发作大鼠同侧的岛叶及杏仁核的脑电图特征的观察,探讨在癫痫发作中岛叶和杏仁核的相互关系。方法:40只SD大鼠分为2组:岛叶组和杏仁核组,各20只大鼠,分别在岛叶或杏仁核植入电极,岛叶组和杏仁核组再根据注射药物的不同分为实验组(10只)和对照组(10只),实验组自药物注射导引管注射0.1mol/L普鲁卡因1μL,而对照组则注射0.1mol/L磷酸盐缓冲液(Phosphated buffered saline,PBS)1μL,建立杏仁核和岛叶电刺激点燃癫痫发作模型。结果:在注射普鲁卡因后,无论在岛叶组还是杏仁核组,实验组的癫痫大发作触发阈值(generalized seizure-triggering threshold,GSTT)明显较注射前升高,存在显著性差异,而在对照组却没有明显的变化。岛叶组注射普鲁卡因前后点燃岛叶GSTT的变化大于杏仁核组GSTT的变化,存在显著性差异。结论:同侧杏仁核和岛叶可以理解为一个"灶性复合体",在这个复合体中,杏仁核起着主导作用,岛叶在杏仁核参与的条件下,是癫痫传播通路的重要组成部分,而杏仁核同样在岛叶所诱发的癫痫发作的传播中为不可或缺的结构。
