群体手骨关节炎严重程度判定指标的建立

目的　建立一个比较不同地区人群骨关节炎严重程度的指标 ,即群体诊断指标 (OAP) ;方法　拍成人手部X线片 ,按照关节间隙狭窄、软骨下硬化、骨赘形成、囊样变等打分 ,相加后与患病率相乘 ,建立一个新指标(OAP)。这个指标考虑到两个方面 :一是骨关节炎患病率 ,另一是严重程度 ,避免了用个体判定代替群体判定造成的偏差 ;结果　利用OAP指标对不同地区成人骨关节炎严重程度进行判别 ,结果令人满意 ,可以直观、定量地表示出某地骨关节炎的严重程度 :<5 0 0分为病情轻度病变人群 ;5 0 0～ 9 99分之间为中度病变人群 ;10 0 0～ 14 99之间为病情重度人群 ;>15 0 0分为病情极重人群 ;结论　OAP指标适用于人群骨关节炎患病程度的判定 ,是一个较好的流行病学群体诊断指标。

The establishment of population hand osteoarthritis severe degree index

Object: To create a index about evaluating the severe degree of OA for population over 40 years old. Methods: According to the severe degree of joints changes of right hand showed in the radiograph (joint space narrowing, sub-chondral sclerosis, cyst and osteophyte), we graded for each person, then we could get a sum of all the grades. Through multiplying the sum with prevalence rate, a new index called OAP would be created. OAP comprises prevalence rate and severe degree of joint changes, and may avoid the bias from applying individual index only. Results: OAP may assess osteoarthritis severity in different populations directly and quantificationally. If population OAP value was lower than 5.00, then it was thought a mild OA population; if OAP value was between 5.00 and 9.99, it was thought a moderate OA population; if OAP value was between 10.00 and 14.99, it was thought a severe OA population; and if OAP value was higher than 15.00, then it was thought terribly severe OA population. Conclusion: OAP is a good diagnosis index for epidemiologic study of population osteoarthritis.
