磁共振成像在宫颈癌诊断中的应用价值分析

目的分析宫颈癌的磁共振成像(MRI)表现,探讨MRI在宫颈癌分期的应用价值。方法收集我院52例宫颈癌患者在治疗前行MRI检查,观察肿瘤的位置、大小、信号特征及宫旁浸润情况,进行影像学分期,并与临床分期、病理分期对比,评价MRI分期价值。结果 4例ⅠA期宫颈癌MRI检查未见病灶,48例ⅠB以上宫颈癌MRI检查均显示病灶,分期分别为ⅠB 26例,ⅡA 11例,ⅡB 6例,ⅢA 3例,ⅢB 1例,ⅣA 1例,43例与病理分期相一致,敏感度为92%,特异度为90%,总正确率为83%。结论MRI对ⅠB期以上宫颈癌病灶均可发现,能够清晰显示阴道受累、宫旁浸润及淋巴结转移的情况,对宫颈癌的诊断及分期的准确率明显高于临床,有较高的应用价值。

Analysis of the clinical value of magnetic resonance imaging in cervical cancer

Objective To analyze the magnetic resonance imaging(MRI) features of cervical carcinoma,and explore the value of MRI in the staging of uterine cervical carcinoma.Methods Fifty-two cases with cervical carcinoma were collected from our hospital before MRI examination which were observed the tumor location,size,signal character-istics and parametrial infiltration.MRI findings were compared with clinical staging and surgical pathologic findings to evaluate the value of MRI imaging in the staging.Results There were 4 cases of ⅠA period of cervical cancer that MRI examination did not show lesions.There were 48 cases of ⅠB period above of cervical cancer that MRI inspection demonstrated the focus of infection.The stage were as follows: ⅠB(26 cases),ⅡA(11 cases),ⅡB(6 cases),ⅢA(3 cases),ⅢB(1 cases),ⅣA(1 cases).There were 43 cases identical with pathological staging.The sensitivity and specificity for parauterine extension were 92% and 90%.The total accuracy of MRI staging was 83%.Conclusion MRI examination can find all the cases of ⅠB period above of cervical cancer,clearly show that vaginal involvement,parametrial infiltration and lymph node metastasis.Cervical cancer diagnosis and staging accuracy rate is significantly higher than clinica,MRI examination has the high application value.
