联合磁共振成像多序列成像在诊断脑弥散性轴索损伤中的价值

目的 探讨联合磁共振成像(MRI)多序列成像在诊断脑弥散性轴索损伤(DAI)的价值,优化MRI检查DAI患者的常规序列.方法 29例临床诊断为DAI的患者均在伤后12～72 h内行MRI检查,采用常规自旋回波T1加权像(SE-T1WI)、快速自旋回波T2加权像(TSE-T2WI)、液体衰减反转恢复T2加权像(FLAIR-T2WI)、T2*稳态自由进动梯度回波(T2*-FFE)和弥散加权成像(DWI)(b=1 000)进行轴位扫描,分析、比较其在DAI的MRI表现,探讨各序列在诊断DAI的优缺点.结果在病灶检出率方面,TSE-T2WI明显高于SE-T1WI,差异有统计学意义(P＜0.05);FLAIR-T2WI、T2*-FFE 和DWI(b=1 000)明显高于TSE-T2WI,差异有统计学意义(P＜0.05),而后三者(FLAIR-T2WI、T2*-FFE 、DWI)相差不大,差异无统计学意义(P＞0.05).结论SE-T1WI、TSE-T2WI、FLAIR-T2WI、T2*-FFE 和DWI(b=1 000)各序列在显示DAI病灶方面各有优缺点,应结合起来作为MRI检查DAI患者的常规序列.

The value of magnetic resonance imaging multiple sequence combined application in diagnosis of cerebral diffuse axonal injury

Objective To study the value of magnetic resonance imaging(MRI) multiple sequence in diagnosis of cerebral diffuse axonal injury(DAI),and to optimize MRI general sequence in examination of patients with DAI.Metho-ds Twenty-nine patients with DAI were examinated by MRI between 12 and 72 hours after being injured,using SE-T1WI,TSE-T2WI,FLAIR-T2WI,T2*-FFE and DWI(b=1 000) for axial scanning to analyze their MRI performance of DAI and compare the advantages and disadvantages of different sequence in diagnosis of DAI.Results In the detection of lesion,TSE-T2WI were significantly higher than SE-T1WI(P<0.05),FLAIR-T2WI,T2*-FFE and DWI(b=1 000) were significantly higher than TSE-T2WI(P<0.05),while the latter three(FLAIR-T2WI,T2*-FFE and DWI) had little difference(P>0.05).Conclusion SE-T1WI,TSE-T2WI,FLAIR-T2WI,T2*-FFE and DWI(b=1 000) have their own advantages and disadvantages in the display of DAI lesions,which should be combined as a general MRI sequence in examination of patients with DAI.
