颅内肿瘤立体定向放射外科治疗后病理形态研究

摘    要:观察颅内肿瘤立体定向放射外科(SRS)治疗后肿瘤及周围脑组织的病理改变,材料与方法对18例颅内肿瘤SRS治疗后手术治疗的标本进行了光镜及电镜下病理形态的研究。结果瘤组织的凝固性坏死在SRS术后11天即可出现,但与肿瘤性质等因素有关。瘤内小血管的辐射损伤不仅在SRS的远期效应,在急性期瘤组织杀伤效应中也起重要的作用。肿瘤周围脑组织病理变化提示存在放射性反应。结论(1)SRS应以最小剂量获得最大治疗效果为原则。(2)SRS治疗后改善脑组织血液循环和促进小血管及髓鞘结构的修复再生,对减少并发症可能有重要意义。

Abstract:Purpose To observe the histo - pathologic changes of tumor and surrounding tissue on intracranial tumor after stereotactic radiosurgery. Materials and Methods 18 intracranial tumor samples after steccotactic radiosurgery ( SRS) were observed morphologically by light and electron microscopy , Results The results showed that the coagulative necrosis of tumor tissue could be observed on 11 days after SRS and this phenomena tended to be closely related to the differentiation of the tumors Small vessel radioreaction within the tumor tissue not only played an important role in chrunicle but acute effects on killing tumor cells after SRS as well . The pathological changes of the surrounding brain tissue showed radiation brain injury occurred in some patients . The relationship between the complication and pathological changes of intracranial tumor after SRS was also discussed. Conclusion ln order to obtain the optimum result, managements of improving microcirculation may benefit to SRS for intracranial tumor .
