新疆哈萨克族同型半胱氨酸与原发性高血压患者早期肾功能损害的相关性研究

摘    要:目的探讨新疆哈萨克族原发性高血压患者同型半胱氨酸与早期肾功能损害的相关性。方法选取参与流行病学调查的新疆哈萨克族原发性高血压患者465例,根据同型半胱氨酸(Hcy)水平分组:Hcy≥10μmol·L-1分为H型组(240例),Hcy<10μmol·L-1为单纯组(225例)。检测2组患者的尿酸(UA)、低密度脂蛋白胆固醇(LDL-C)、Hcy、尿素氮(BUN)、血肌酸酐(SCr)和胱抑素C(CysC)水平,比较2组基线资料及肾功能相关指标,用二分类Logistic回归分析早期肾损害的危险因素。结果H型组和单纯组的收缩压(SBP)分别为(146.5±21.3)和(140.5±19.4)mmHg;舒张压(DBP)分别为(93.9±13.8)和(88.1±14.8)mmHg;UA分别为(270.0±78.1)和(202.2±69.7)μmol·L-1;LDL-C分别为(3.1±0.8)和(2.8±0.9)mmol·L-1;Hcy分别为15.5(12.9,18.2)]和8.4(7.4,9.2)]μmol·L-1;BUN分别为(5.6±1.3)和(5.1±1.4)mmol·L-1;SCr分别为(82.6±17.0)和(68.2±11.9)μmol·L-1;CysC分别为(1.1±0.2)和(0.8±0.3)mg·L-1。组间比较,上述指标的差异均有统计学意义(均P<0.01)。Logistic回归分析显示,LDL-C、Hcy和CysC是哈萨克族原发性高血压患者早期肾损害的危险因素。结论新疆哈萨克族原发性高血压患者Hcy水平的升高参与了早期肾损害的过程,降低Hcy水平可延缓高血压患者肾功能损害进程。

Abstract:Objective To investigate the correlation between homocysteine and early renal impairment in patients with essential hypertension in Kazakhs in Xinjiang.Methods A total of 465 Kazakh patients with essential hypertension in Xinjiang who participated in the epidemiological survey were selected.The patients were divided into two groups according to the level of homocysteine:H-type group(Hcy≥10μmol·L-1,240 cases)and pure group(Hcy<10μmol·L-1,225 cases).The uric acid(UA),low-density lipoprotein cholesterol(LDL-C),homocysteine(Hcy),urea nitrogen(BUN),serum creatinine(SCr),cystatin C(CysC)and other related indicators of each patient were detected.Baseline data and related parameters of renal function were compared between two groups.The risk factors of early renal impairment were analyzed by the binary classification logistic regression.Results The systolic blood pressure(SBP)of the H-type group and the pure group were(146.5±21.3),(140.5±19.4)mm Hg;the diastolic blood pressure(DBP)were(93.9±13.8),(88.1±14.8)mm Hg respectively;UA were(270.0±78.1),(202.2±69.7)μmol·L-1;LDL-C were(3.1±0.8)and(2.8±0.9)mmol·L-1,respectively;Hcy were15.5(12.9,18.2)],8.4(7.4,9.2)]μmol·L-1,respectively;BUN were(5.6±1.3),(5.1±1.4)mmol·L-1;SCr were(82.6±17.0),(68.2±11.9)μmol·L-1,respectively;CysC were(1.1±0.2)and(0.8±0.3)mg·L-1,respectively.The differences of the factors were statistically significant(all P<0.01).Logistic regression analysis showed that LDL-C,Hcy and CysC were risk factors for early renal impairment in Kazakhs patients with essential hypertension.Conclusion The increase of Hcy level in Kazakh patients with essential hypertension in Xinjiang is involved in the early process of renal damage,and the decrease of Hcy level may delay the progression of renal impairment in patients with hypertension.
