手法旋转胎头联合扩张宫颈术在产程中的作用观察

目的探讨手法旋转胎头联合扩张宫颈术在产程中的作用及对分娩的影响。方法随机抽取120例头位分娩的产妇,在第一产程中因活跃期宫颈水肿或胎方位异常而使用手法旋转胎头联合手法扩张宫颈术者作为观察组,另取同等条件的产妇作为对照组,不予任何干预。结果2组自然分娩率、剖宫产率、活跃期平均时间、第二产程平均时间、宫颈扩张及胎头下降速度有显著性差异,而产后出血量、宫颈裂伤以及胎儿新生儿的损伤情况无显著性差异。同时对此方法的时机进行了观察,结果是宫口开大5~8 cm时为最佳时机。结论手法旋转胎头联合扩张宫颈术可缩短产程,降低剖宫产率。

Observation of the function of hand revolving fetal head combined with expanding cervix operation in birth process

Objective It is to explore the function of hand revolving fetal head combined with expanding cervix operation in birth process and its effect on childbirth.Methods 120 lying-in women with head position were randomly chosen as observation group to be performed on with hand revolving fetal head combined with expanding cervix operation because of cervix edema or abnormal fetal position in active stage in the first birth process.Another group of women with the same condition was chosen as control group without any intervention.Results There was significant differences in spontaneous delivery rate,uterine-incision delivery rate,average times of active stage and the second birth process,and the speeds of cervix expanding and fetal head lowing,and no obvious differences in blood loss and injury of cervix and infants after delivery.It was the best time to use the operation when the cervix opening expanded to 5-8 cm.Conclusion Hand revolving fetal head combined with expanding cervix operation can shortened the birth process and decrease the rate of uterine-incision delivery.
