声带良恶性增生疾病噪音的声学参数分析

摘    要:目的:观察研究正常及声带良恶性增生疾病噪音声学和持征,探讨计算机声学测试技术在喉疾病鉴别诊断及预后观察中的应用价值。方法:采用计算机噪音声学测试系统对80例正常成人及140例声带良恶性增生疾病患者元音信号进行检测,对频率微扰(jitter),振幅微扰(shimmer),声门噪声能量(NNE)等进行分析。结果:140例病态噪音均表现一项或几项声学参数值高于正常,各参数均值较正常值判别有统计学意义。声带良性增生组jitter值大于正常者占24%,shimmer占83%,NNE占58%,各参数均值较正常值约升高1-1.5倍左右。声带不典型增生组jitter,shimmer,NNE值高于正常者分别占87%,94%,90%,各参数均值较正常值升高4.5-5倍左右。结论:计算机声学分析各项参数可作为噪音定量评价的客观指标,有助于喉部病变性质的初步鉴别及动态观察预后。

Abstract:Objective To investigate the characteristics of acoustic parameters of benign and malignant diseases of vocal cords and discuss the value of acoustic analysis technique by computer in the objective differentiation and prognosis of the laryngeal diseases.Methods The voice signals of sustained vowel“a” were measured with a micro-computer in 80 healthy adults and 140 patients with varying benign and malignant diseases of vocal cords.The acoustic parameters (jitter,shimmer and NNE) were analyzed and compared.Results The acoustic parameters were significantly increased in all patients with benign and malignant diseases of vocal cords.The mean parameters were 1-1.5 times in benign diseases,1-2.5 times in atypical hyperplasia,and 4.5-5 times in carcinoma higher than the control group.Conclusion The acoustic parameters can be used as objective standards for evaluating voice function quantitatively.The acoustic analysis by computer is of value in the objective differentiation and prognosis observation of the laryngeal diseases.
