不同品牌3MP影像诊断显示器对孤立性肺结节伴随征象及性质判读的初步研究

目的 评价不同品牌3百万像素(MP)的影像诊断显示器对胸部数字化X线摄影(DR)影像上肺结节伴随征象及定性诊断的效能.方法 利用的3种不同品牌显示器,分别为台湾的奇菱CHILIN MDM2130-3NC(品牌A)、日本的艺卓EIZO Radi-Force G31(品牌B)、美国的平达PLANAR Dome E3n(品牌C),分辨率为(1536×2048)20.8英寸医用竖屏单色LCD.胸部DR中发现有肺结节者行CT检查,并经CT证实为单发(10～30 mm直径)的肺结节40例.从8个方面对结节伴随征象进行对比,得出的数据进一步对肺部结节进行初步定性诊断.判定各品牌显示器对结节伴随征象及定性诊断的符合率.结果 品牌B的K值有5项超过了0.40,品牌A、C的K值各有2项超过了0.40;从8种分类比较得出,3MP的显示器对于钙化、胸膜尾征及空洞、空腔的符合率均达到了0.8以上,对于这些征象可以说DR有较高的识读能力;而对于边缘、密度、分叶及毛刺征,3种显示器的符合率明显降低,最低达到了0.575.结论 本研究所涉及的显示器中,EIZO在孤立性肺结节周边伴随征象的识读方面稍强于其他2个品牌.但总的来说,DR胸片对于肺部结节病灶伴随征象及定性准确率不高.

Preliminary Study of Accompanied Signs and Qualitation of Solitary Pulmonary Nodules by using 3MP Images Diagnostic Monitors with Different Brand

Objective To evaluate the value of 3 million-pixel(MP) imaging diagnostic monitors with different brand in qualifying diagnosis of accompaniment signs of pulmonary nodule on the chest digital radiography (DR). Methods Three different brands of monitors were used in this study,including Taiwan's CHILIN MDM2130 -3NC(brand AJ ,Japan EIZO RadiForce G31(brand B) and the United States PLANAR Dome E3n( brand C), the resolution was all (1536 × 2048), and the monitors were all 20. 8 inch monochrome screen erected medical LCD,40 cases with solitary pulmonary nodules(SPN) detected by DR and the nodules with 10～ 30 mm in diameters showed on CT images were involved in this study. 8 signs of SPN on DR and CT were analysed,based on which, the qualifying diagnosis of SPN was done,and the accurate rates of 8 signs of SPN and the qualifying diagnosis of SPN were evaluated and compared between three different monitors. Results The K values of 5 signs were more than 0. 40 in brand B, while only 2 signs were more than 0. 40 in brand A and C. The corresponding rates of calcification,pleural tail sign and cavity showed by 3MP reached more than 0. 8, it suggested that DR had high reading capability for these signs, and for the other signs, DR had lower reading capability, the lowest was 0. 575. Conclusion In showing the accompanied signs of SPN,EIZO are better than that of the other two brands, but overall,chest DR in showing the accompanied signs of pulmonary nodule and qualifying diagnosis of SPN in not satisfactory.
