颅脑外伤术后颅内感染的相关因素和有效治疗

摘    要:目的探讨颅脑外伤术后颅内感染的相关因素及有效治疗。方法选择于2009年3月—2014年5月在我院因颅脑外伤行手术治疗后发生颅内感染的患者61例,对所有患者的临床资料进行分析,并探讨其临床治疗方法。结果通过分析总结,可见颅脑外伤术后发生颅内感染与患者术前GCS评分、颅脑损伤类型、手术持续时间、手术治疗次数、切口脑脊液漏以及低蛋白血症等因素有关,差异具有统计学意义(P〈0.05);而与性别、年龄及气管是否切开等因素无关,差异无统计学意义(P〉0.05)。将单因素分析中具有统计学意义的6项进行Logistic多元回归分析,发现切口脑脊液漏、手术次数、手术时间及低蛋白血症是颅脑外伤术后发生颅内感染的独立高危因素,差异均具有统计学意义(P〈0.05)。经治疗,痊愈49例(80.33%),治疗无效死亡12例(19.67%)。结论颅脑外伤术后发生颅内感染的高危因素众多,采取积极有效的预防性措施,及时给予合理的抗菌药物,是提高治愈率的关键。

Abstract:Objective Factors Related to traumatic brain injury and intracranial infection after effective treatment. Methods Select the 61 patients in March 2009 to May 2014 in our hospital after surgical treatment of traumatic brain injury due to intracranial infection. The clinical data were analyzed for all patients, and the clinical treatment. Results By analyzing the summary, showing the former head trauma and postoperative infection in patients undergoing intracranial GCS score, brain injury type, duration of surgery, surgical treatment times, cut cerebrospinal fluid leakage and low albumin and other factors, the difference was statistically significance (P〈O.05); but not with sex, age and other factors unrelated to the trachea are cut, the difference was not statistically significant (P〉0.05). The six were Logistic regression analysis was statistically significant in univariate analysis, we found incision cerebrospinal fluid leakage, the number of operations, operation time and low albumin is an independent risk factor for the occur- rence of intracranial infection after traumatic brain injury, the differences were statistically significant (P〈0.05). After treatment, cured 49 cases (80.33%), treatment died 12 cases (19.67%). Conclusion Traumatic brain injury occurred after a number of risk factors for intracranial infection, take positive and effective preventive measures, timely and reasonable antimicrobial drugs, is the key to improve the cure rate.
