空间-时间相关成像技术诊断胎儿室间隔缺损的应用研究

摘    要:目的应用空间-时间相关成像技术(STIC)多种处理模式对胎儿室间隔缺损(VSD)声像学特征进行研究,探讨STIC技术多种模式在评价胎儿VSD中的应用价值,并比较STIC技术多种模式间诊断效能的差异。方法选取疑诊为先天性心脏病胎儿54例,应用常规超声心动图(UCG)和STIC技术多种处理模式分别进行分析,并以产后新生儿心脏超声诊断或引产的胎儿尸体解剖结果为金标准。结果常规超声心动图、断层超声成像和表面成像模式对胎儿VSD的诊断效能较高,ROC曲线下面积三者之间进行比较差异无统计学意义(P>0.05),多平面成像、透明成像和反转成像模式的诊断效能较差。结论 STIC技术多种处理模式虽各具特色,但并不能代替二维超声心动图,联合应用可提供更清晰、更稳定、更丰富的图像内容。可作为常规超声心动图诊断胎儿VSD的最佳补充手段。

Abstract:[Abstract] Objective To investigate the value of five modes of spatio-temporal imageco correlation (STIC) technique and to contrast the disparity of the each mode of STIC in the diagnosis of ventricular septal defect (VSD) of fetus. Methods Fifty-four cases suspected with VSD were diagnosed by UCG and analyzed by five modes of STIC technique. The accuracy of the UCG and the each mode of STIC tecbnique in the diagnosis of VSD of fetus was checked by comparing the results to the autopsy and neonatal echocardiography after they were born. Results The activities of UCG, TUI and Rendering modes were higher than the activities of MP, Glassbody and Inversion modes. There were no significance comparing in the three modes with the areas below the ROC curves. Conclusions STIC can not replay the UCG in diagnosis fetal VSD,but it can provid affluent, limpid and stabile picture informations, and can be the best subsidiary method for UCG.
