眶内血肿28例临床分析

目的探讨眶内血肿的病因、诊断和治疗。方法回顾分析资料完整的28例(28眼)眶内血肿。其中,眼眶血管性病变12例,自发性6例,外伤性5例,手术后并发4例,血小板减少1例。均有不同程度的视力下降、眼球突出、眼球运动障碍及眶压升高。结果2例经急诊药物治疗,7例经眶内穿刺抽血,19例经开眶手术。患眼眼球运动、突出度均有好转,眶压下降,视力部分改善。结论对眶内血肿早期穿刺抽血或开眶手术等效果良好;由眼眶血管性病变等所致者需同时处理原发病变。

Clinical analysis of 28 cases of orbital hematoma

Objective To discuss the etiological factor,diagnosis and treatment of orbital hematoma.Methods 28 cases of orbital hematoma refered to Zhongshan Ophthalmic Center,Sun Yat-sen Univercity from Jan 1,1991 to Mar 31,2003 were analyzed retrospectively.Among them,12 had localized orbital vascular lesions,6 spontaneous,5 traumatic,4 surgical complication and 1 thrombocytopenia.All cases showed impaired vision,proptosis,ocular movement disorder and higher intraorbital pressure.Results 2 cases were treated by medication,7 by orbital puncture and 19 by orbitotomy.All patients had vision improvement,ocular movement recovery and decreased proptosis after theapy. Conclusions Early puncture of orbitotomy had good therapentic effects in pataients with orbital hematoma;For pataients with primary orbital vascular lesions,we should deal with them as well.
