摘    要:目的 基于时间窗动态度中心度(DC)分析方法观察伴中央颞区棘波良性癫痫(BECTS)患儿静息态大脑功能网络改变,探讨其潜在神经生物学机制。方法 对26例临床诊断BECTS患儿(BECTS组)和26名健康儿童(对照组)采集静息态功能磁共振(rs-fMRI)图像,以时间窗动态DC法处理数据,获得二值化和加权DC变异系数(CV),比较组内及组间差异,分析BECTS患儿DC CV存在差异脑区的二值化和加权DC CV与其临床表现的相关性。结果 2组组内二值化和加权DC CV空间分布均存在广泛相似性。相比对照组,BECTS组双侧楔前叶、右侧颞上回二值化DC CV增加、左侧颞下回减低;BECTS组双侧楔前叶、右侧颞上回、左侧丘脑及左侧角回加权DC CV增加,左侧颞下回、右侧梭状回及左侧额下回三角部减低。相关性分析显示,BECTS患儿右侧颞上回二值化DC CV与病程呈负相关(r=-0.664,P<0.005),楔前叶二值化DC CV与总智商呈负相关(r=-0.712,P<0.005),右侧颞上回加权DC CV与言语智商呈负相关(r=-0.673,P<0.005)。结论 BECTS患儿存在网络中央节点改变,尤其异常功能网络节点内高度连接脑区存在选择性及特异性损伤主要涉及语言皮层和默认皮层,可能与发病有关。

Abstract:Objective To observe the changes of brain functional network in childhood benign epilepsy with centrotemporal spikes (BECTS) using dynamic degree centrality (DC) method based on temporal window. Methods Resting-state functional MRI (rs-fMRI) were collected from 26 BECTS children (BECTS group) and 26 healthy children (control group).The data were analyzed using temporal window-based dynamic DC method, and the coefficient of variation (CV) of binary and weighted DC were obtained. The intra- and inter-groups parameters were compared between groups, and the correlation of CV of Binary and weighted DC of BECTS children with statistical differences between groups and clinical parameters were analyzed. Results CV of Binary and weighted DC spatial distribution of BECTS group and control group were broadly similar. In BECTS group, CV of Binary DC increased in bilateral precuneus and the right superior temporal gyrus, but decreased in the left inferior temporal gyrus, whereas of weighted DC in bilateral precuneus, the right superior temporal gyrus, the left thalamus and the left angular gyrus increased, but decreased in the left inferior temporal gyrus, the right fusiform gyrus and the left inferior frontal trianguleris cortex. Correlation analysis showed significant negative correlation between the epilepsy duration and CV value of binary DC in the right superior temporal gyrus (r=-0.664, P<0.005), between the total intelligence and CV value of binary DC in precuneus (r=-0.712, P<0.005), also between the verbal intelligence and CV value of weighted DC in the right superior temporal gyrus (r=-0.673, P<0.005). Conclusion BECTS children had changes in the central nodes of the network, especially the selectivity and specificity of the highly connected brain regions in the abnormal functional network nodes impaired, mainly involving the language cortex and the default cortex, which might be related to the pathogenesis of BECTS.
