小柴胡汤配合西药四联疗法治疗十二指肠球部溃疡临床观察

目的研究十二指肠球部溃疡临床接受小柴胡汤加减配合西药四联疗法治疗的效果。方法随机将2018年1月—2019年2月150例十二指肠球部溃疡患者分为2组。74例为对照组,仅接受西药四联疗法治疗;76例为观察组,在西药治疗基础上联合中药小柴胡汤加减治疗。比较2组胃肠激素水平、治疗有效率、不良反应发生率、溃疡复发率。结果观察组治疗后SS、CGRP高于对照组,GAS、MTL、AM低于对照组(P<0.05);观察组治疗后总有效率为94.74%,高于对照组总有效率83.78%(х2=4.7204,P<0.05);观察组治疗后不良反应发生率为5.26%,明显低于对照组不良反应发生率16.22%(х2=4.7204,P<0.05);观察组半年内溃疡复发率为2.64%,HP感染再发率为1.32%,低于对照组12.16%、8.10%(х2=4.0312、3.8881,P<0.05)。结论小柴胡汤加减配合西药四联疗法治疗十二指肠球部溃疡能够更明显提升效果,且能够保证治疗安全性,减少溃疡复发。

Clinical Observation on Xiaochaihu Decoction Combined with Western Medicine Quadruple Therapy in the Treatment of Duodenal Bulbar Ulcer

Objective To study the effect of Xiaochaihu decoction and Western medicine quadruple therapy on duodenal bulbar ulcer.Methods 150 patients with duodenal bulbar ulcer from January 2018 to February 2019 were randomly divided into two groups.74 cases in the control group were only received Western medicine quadruple therapy,and 76 cases in the observation group received Xiaochaihu decoction and Western medicine quadruple therapy.The levels of gastrointestinal hormones,treatment efficiency,adverse reactions and recurrence rate were compared between the two groups.Results After treatment,SS and CGRP in the observation group were higher than those in the control group,GAS,MTL and AM in the observation group were lower than those in the control group(P<0.05).The total effective rate in the observation group was 94.74%,which was higher than 83.78% in the control group(χ^2=4.7204,P <0.05).The incidence of adverse reactions in the observation group was 5.26%,which was significantly lower than that in the control group(16.22%)(χ^2=4.7204,P <0.05).The ulcer recurrence rate in observation group was 2.64%,HP infection recurrence rate was 1.32%,which was lower than that in the control group(12.16%,8.10%) respectively(χ^2=4.0312,3.8881,P <0.05) Conclusion Xiaochaihu decoctionand Western medicine quadruple therapy for duodenal bulbar ulcer can significantly improve the effect,and can ensure the safety of treatment and reduce the recurrence of ulcer.
