肝脏铁过度沉着的磁共振成像

摘    要:目的:描述肝脏铁过度沉着的磁共振表现,对原发性和继发性铁过度沉着进行鉴别。材料与方法:对8例铁过度沉着患者进行肝脏MRI扫描,并对T2加权像(T2WI)进行回顾性分析。结果:T2WI对病变的显示较敏感。无论何种病因,肝脏均呈均匀性低信号;而胰腺和脾脏则依病因不同,分别呈现低信号。结论:MRI对肝脏等器官的铁过度沉着诊断较为准确,将MRI影像与临床资料相结合,能够鉴别原发性和继发性铁过度沉着,在加深对本病的认识、协助治疗及判断预后等方面有较大意义。

Abstract:Purpose:TodescribetheMRfindingsinliverironoverload,andtodiferentiatetheprimary(parenchymal)fromthesecondary(reticuloendothelial)ironaccumulation.Materialsandmethods:HepaticMRimagingwasperformedin8patientswithclinicalyprovedliverironoverload.T2-weightedMRimageswereretrospectivelyanalysed.Results:T2-weightedMRimageswereverysensitiveinshowingthesignalchangescausedbyirondeposition.Theliverdemonstratedhomoge-neouslowsignalinal8casesregardlessoftheunderlyingcauses,whereasinextrahepaticorgans(pancreasandspleen),lowsignalintensitiesoccurredindiferentconditions.Conclusion:MRimag-ingisnotonlyaccurateinmakingthediagnosisofliverironoverload,butalsoveryusefulindistin-guishingtheprimaryfromthesecondaryhepaticironaccumulationwhencombinedwiththeclinicalhistory.MRimagingisanidealimagingmethodprovidingadditionalinformationofthedisorderandishelpfulinplanningthetherapyandevaluatingtheprognosis.
