风心病肺动脉高压患者围术期呼吸指数变化

目的探讨风湿性心脏病(风心病)肺动脉高压患者围术期呼吸指数的变化及其临床意义.方法将60例风心病患者分为肺动脉高压组(30例)、对照组(30例),对其围术期呼吸指数变化进行分析比较.结果肺动脉高压组呼吸指数术后2 h降至较低水平12 h后又逐渐增高,在术后18 h达到高峰后逐渐下降,24 h基本达到术前水平,呈明显不稳定状态,而对照组则相对平稳.结论风心病肺动脉高压患者术后24 h内肺的换气功能处于不稳定状态,适当延长呼吸机辅助呼吸时间有利于渡过术后危险期.

Measurement of respiratory indices perioperative of rheumatic heart diseases with pulmonary hypertension

Objective To observe the postoperative respiratory character of the patients with rheumatic heart diseases and pulmonary hypertension, and their clinical significance. Methods All patients with pulmonary hypertension were divided into pulmonary hypertension group (30 patients) and control group(30 patients) to be analysed for their respiratory index changes at different times in the operation. Results The respiratory index reduced to the lowest level at the second postoperative hour and then, gradually increased to the peak at the 18th postoperative hour. Thereafter, it reduced gradually again. At the 24th postoperative hour it returned to the preoperative level.Comparatively the control group did not showed fluctuation of the indices. Conclusion The patients with pulmonary hypertension have an unstable respiratory function in the first 24 hours after the operation. A longer time mechanical ventilation is helpful for the patients to pass this dangerous period safely.
