螺旋CT及其三维重建在儿童重度肾及输尿管损伤中的应用

摘    要:目的 探讨螺旋CT及三维重建图像在小儿重度肾,输尿管损伤中的应用价值。方法 对4例车祸伤患儿,应用PickerCTTwin扫描双肾,输尿管区,包括平扫、增强后实质期和分泌期扫描。采用MPR、MIP和3D对图像进行后处理。结果 2例示肾裂伤并UPJ部断裂,1例肾下极完全性断裂并肾盂破裂,1例为单纯性UPJ部断裂,4例均见有巨大尿囊形成,手术证实CT诊断。结论 螺旋CT可准确和清晰地显示肾损伤的程度和范围,三维重建像从多个角度,立体显示肾损伤的全貌及与周围结构的关系。图象直观,立体感强,为临床诊断和治疗提供了可靠依据。

Abstract:Objective To evaluate spiral CT and three dimensional reconstruction in the diagnosis of severe renal trauma in children. Methods 4 children were taken spiral CT one month after injured in car accidents.Picker CT Twin is used to scan kidney and ureter,including plane scan and contrast enhancement nephrography. Pitch 1.0 and slice thickness 6.5 mm were selected.MPR,MIP and 3D reconstruction were used as postprocessing. Results Of the 4 cases,2 cases had laceration of right kidney and UPJ avulsion,1 case a completely amputated lower pole of left kidney and 1 case a UPJ avulsion in right ureter. Giant urine cysts could be found in all the 4.The CT findings were confirmed by operations. Conclusions Spiral CT and 3D images could accurately depict the condition and severity of injuries of kidney and ureter.The 3D images depict the injuries and surounding structures from various directions,so that the optimal kind of operation could be selected with the reference of images.
