米索前列醇联合缩宫素防治产后出血疗效观察

目的探讨米索前列醇联合缩宫素防治产后出血的疗效。方法160例足月阴道分娩产妇按照入院先后的顺序随机分为观察组和对照组各80例，对照组在胎头娩出后立即给缩宫素10U，肌肉注射或静滴，观察组在缩宫素治疗的基础上于直肠肛塞放置米索前列醇200μg。观察比较两组第三产程时间的持续时间、产后2h出血量、产后24h出血量以及副反应情况。结果两组产后出血的发生率比较及第三产程的持续时间比较，差异有显著性（P〈0．05），且术后2h平均出血量及24h平均出血量分别与对照组比较，差异有显著性（P〈0．05）。结论二者联合应用降低了产后出血发生率和产后出血的出血量，副作用少，临床疗效优于单纯使用缩宫素，值得临床推广和应用。

Studies on the Effect of Misoprostol Combined with Oxytocin in Prevention and Treatment of Postpartum Hemorrhage

Objective To investigate the effect of misoprostol combined with oxytocin in prevention and treatment of postpartum hemorrhage. Methods There were 160 cases of full term vaginal delivery and maternal hospital has in accordance with the order randomly divided into two groups： the observation group and the control group, 80 cases in each. After the fetal head was delivered immediately, the control group was administrated with oxytocin 10U by intramuscular injection or by intravenous infusion. On the basis of oxytocin treatment, the observation group was administrated with misoprostol in dosage of 200μg in rectal anal plug. Duration of third stage labor, blood loss of 2h, 24h postpartum hemorrhage, and side effects were observed. Results For incidence of postpartum hemorrhage and the duration of third stage of labor, the dif- ference was significant（P〈0.05 ）, and the average amount of blood loss of 2h, 24h postpartum hemorrhage had significant difference compared with the control group （P〈0.05）. Conclusion The combination of misoprostol and oxytocin can reduce both the incidence of postpartum hemorrhage and blood loss, has fewer side effects and better clinical efficacy than the use of oxytocin alone, thus it is worth of wide use in clinic.
