临床分离表皮葡萄球菌生物膜形成与其耐药性的相关性分析

摘    要:目的分析临床分离的表皮葡萄球菌对抗菌药物的耐药性与生物膜形成能力的关系。方法收集成都地区两家医院非重复临床分离表皮葡萄球菌45株,用半定量黏附实验检测其生物膜形成能力,采用K-B法以统一方案进行抗菌药物药敏试验,按CLSI2009版判断结果,用χ2检验分析表皮葡萄球菌耐药性与生物膜形成能力之间的关系。结果 45株表皮葡萄球菌中18株为生物膜形成株,其中12株来源于分泌物标本,6株来源于其他无菌体液。来源于分泌物和其他无菌体液的表皮葡萄球菌生物膜形成能力差异无统计学意义(P〉0.05);45株表皮葡萄球菌除对万古霉素、利奈唑胺、喹奴普丁/达福普丁有较高的抗菌活性外,对青霉素、氨苄西林/克拉维酸、苯唑西林、头孢唑啉、复方新诺明、林可霉素、环丙沙星、红霉素、庆大霉素、亚胺培南的耐药率均〉50%,特别是苯唑西林耐药表皮葡萄球菌(MRS)高达88.9%.可形成生物膜和不可形成生物膜的菌株对抗菌药物耐药率差异无统计学意义(P〉0.05)。结论表皮葡萄球菌对抗菌药物的耐药性与其生物膜形成能力无相关性。

Abstract:Objective To analyze the correlation between biofilm forming ability and drug resistance of Staphylococcus epidermidis clinical isolates.Methods The biofilm phenotype of 45 strains was detected by semi-quantitative adherence assay.The antimicrobial susceptibility was tested according to an agreed protocol using K-B method.Results were analyzed according to CLSI criteria 2009.The correlation of antibiotic resistance and biofilm formation was analyzed by χ2 test.Results Biofilm phenotype was identified in 18 strains of Staphylococcus epidermidis.Among 18 strains,12 strains were from secretion and 6 strains were from abacterial body fluid.The ability of biofilm formation in strains from secretion and abacterial body fluid was not statistically different(P0.05).Vancomycin,linezolid and quinupristin/ dalfopristin were still highly active against 45 strains.More than 50% of Staphylococcus epidermidis isolates were resistant to penicillin,ampicillin-clavalanic acid,oxacillin,cefazolin,compound sulfamethoxazole,lincomycin,ciprofloxacin,erythromycin,gentamycin and imipenem,especially MRS in Staphylococcus epidermidis was 88.9%.The different resistance rates of strains between biofilm formation and biofilm unproduction were not statistically significant(P0.05).Conclusion No correlation is found between biofilm formation and antibiotic resistance in these isolates by statistical analysis.
