多层螺旋CT灌注成像在诊断甲状腺病变中的应用

摘    要:目的探讨多层螺旋CT灌注成像对甲状腺病变的诊断价值。方法对21例正常甲状腺、32例甲状腺癌、69例良性病变(甲状腺瘤31例,结节性甲状腺肿38例)行MSCT灌注成像,用CT perfusion2软件分析灌注参数血流量(BF)、血容量(BV)、平均通过时间(MTT)和表面通透性(PS)的平均值,并进行统计学分析。结果正常甲状腺组的BF、BV、MTT和PS分别为(549.61±187.91)mL·100g^-1·min^-1、(9.34±3.41)mL/100g、(2.65±0.97)s、(36.07±25.11)mL·100g^-1·min^-1;甲状腺癌组分别为(326.81±139.14)mL·100g^-1·min^-1、(7。95±5.65)mL/100g、(3.90±2.60)S、(54.06±22.95)mL·100g^-1·min^-1;良性病变组分别为(330.53±144.02)mL·100g^-1·min^-1、(8.14±5.33)mL/100g、(3.87±2.56)S、(41.84±24.54)mL·100g^-1·min^-1。3组间比较,BF、PS值差别具有统计学意义性(P=0.001,0.018),BV、MTT值差别无统计学意义(P=0。586,0.093)。各组BF值两两比较。正常组与良恶性组BF值差别具有统计学意义(P=0.000,0.001),良恶性组差别无统计学意义(P=1.000)。PS值恶性组与正常组、良性组差别具有统计学意义(P=0.009,0.020),正常组与良性组差别差别无统计学意义(P=0.341)。结论MSCT灌注成像中灌注参数PS值对甲状腺病变的良恶性判断有一常价信.
