摘    要:目的:探讨鼠神经生长因子(mouse nerve growth factor,m NGF)在糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)临床治疗中的应用价值。方法:选取2型糖尿病合并周围神经病变患者63例,随机分为治疗组(n=32)和对照组(n=31)。对照组口服甲钴胺胶囊,治疗组在此基础上加用鼠神经生长因子。比较两组临床疗效,观察治疗前后神经传导速度及多伦多临床评分系统(TCSS)评分的变化。结果:治疗组总有效率为93.75%,高于对照组的70.97%(P<0.05)。治疗后,两组神经传导速度较治疗前加快(P<0.01),且治疗组加快程度更大(P<0.01)。治疗后,两组感觉、反射、症状评分及总分均较治疗前降低(P<0.05或P<0.01),且治疗组降低程度更大(P<0.05或P<0.01)。治疗期间,两组均未见严重不良反应发生。结论:m NGF联合甲钴胺治疗DPN可改善神经传导功能,加速周围神经病变的恢复,疗效确切,安全性高。

Abstract:Objective:To explore the effect of mouse nerve growth factor (mNGF) in clinical treatment of diabetic peripheral neuropathy (DPN).Methods:A total of 63 patients with type 2 diabetes mellitus (T2DM) combined with peripheral neuropathy were selected and randomly divided into treatment group (n=32) and control group (n=31).Control group was treated with mecobalamin capsules,on which basis treatment group was added with mNGF.Clinical efficacy was compared,and the changes of nerve conduction velocity (NCV) and toronto clinical scoring system (TCSS) before and after treatment were observed in two groups.Results:The total response rate was 93.75% in treatment group,evidently higher than the 70.97% in control group (P<0.05).After treatment,NCV increased markedly in both groups before treatment (P<0.01),which was markedly higher in treatment group than that in control group (P<0.01).After treatment,sensation score,reflection score,symptom score and the total score decreased obviously in both groups than treatment before (P<0.05 or P<0.01),which decreased more significantly in treatment group than that in control group (P<0.05 or P<0.01).During treatment,none had severe adverse reactions in both groups.Conclusion:mNGF combined with mecobalamin can not only improve the nerve conduction function of patients with DPN,but also increase the recovery of peripheral neuropathy,with excellent efficacy and high safety obtained.
