超声引导下与传统股静脉置管术在上腔静脉综合征患者中的置管效果对比

目的:对比超声引导下与传统股静脉置管术在上腔静脉综合征患者中的置管效果。方法:100例上腔静脉综合征患者随机分为两组,超声组患者(50例)在超声下行股静脉穿刺,盲穿组(50例)行盲穿治疗。结果:超声组50例X线拍片有48例穿刺成功,1例误入动脉,1例未进入静脉,1例导管尖端反折经调整后到达下腔静脉,置管期间均未发生大血肿、静脉炎;盲穿组9例失败,其中误入动脉3例,未进入静脉2例,局部大血肿2例,2例因体位及时间过长确实不耐受而放弃继续置管。两组穿刺时间及穿刺成功率比较均有显著统计学差异(P<0.05)。结论:与盲穿股静脉置管术比较,超声引导下股静脉置管能显著提高穿刺成功率及缩短穿刺时间,安全性更好。

The comparison of clinical effect between traditional femoral vein catheterion and ultrasound-guided veni puncture technique on SVCS patients

Objective:Making a comparison of the effect between traditional femoral vein catheter and ultrasound-guided veni puncture technique on superior vena cava syndrome (SVCS) patients.Methods:100 cases of SVCS patients were randomly divided into two groups with the ultrasound-guided group (50 cases) with the femoral vein puncture under ultrasound and the traditional group (50 cases) with blind puncture.Results:Under X-ray,48 cases in 50 achieved successful guiding by ultrasound,and 1 case was guided into the artery by mistake and 1 case was not guided into the vein;1 case of the 48,catheter tip was adjusted to the inferior vena after its reflex,when there was no hematoma or phlebitis during the cathetering.In the other group 9 cases failed,among which 3 cases got into the artery by mistake,2 cases did not get into the vein and 2 cases had local hematoma,2 cases abandoned the treatment because of the position and intolerable long-duration operation.Compared with the puncture time and success rate of the two groups there exists significant statistical difference (P<0.05).Conclusion:Compared with the traditional treatment,the treatment under the guidance of ultrasound can significantly improve the success rate of puncture and shorten the puncture time with higher security.
