摘    要:目的:探讨神经导航和术中超声技术在脑动静脉畸形(AVMs)手术中的应用价值。方法应用神经导航结合术中超声技术显微切除AVMs 12例,Spetzler-Martin分级:2级3例,3级7例,4级1例,5级1例。结果全部病灶均完全切除,术后神经功能改善8例,神经功能同术前2例,癫痫发作1例,轻偏瘫1例;1例自动出院。随访6个月~4年未见血管畸形残留或复发。结论神经导航结合术中超声技术可明显提高AVM的全切除率,改善患者预后。

Applications of neuronavigation combined with intraoperative ultrasound in microneurosurgery of cerebral arteriovenous malformations

Abstract:Objective To investigate the value of neuronavigation combined with intraoperative real ultrasonagraphy in microsurgery of AVMs.Methods Twelve patients with AVMs were operated on with the aid of neuronavigation combined with intraoperative ultrason-agraphy.According to Spetzler-Martin grade,grade II was in 3 cases,grade III in 7 cases,grade IV in 1 case,and grade V in 1 case.Results All malformational angioma nidus were completely removed.Postoperatively,neurological status was improved in 8 cases,and the same as the preoperation in 2 cases.Epileptic seizure was in 1 case,mild hemiparesis in 1 case,and auto-discharge in 1 case because of severe pulmo-nary infection.No mortality was found in all cases.There were no residual or recurrent AVMs after 6 months to 4 years follow-up.Conclusion Neuronavigation combined with intraoperative ultrasound can not only increase the rate of complete resection in AVMs,but also improve the prognosis of the patients.
