多层螺旋CT对寰枢椎病变的诊断价值

摘    要:目的评价多层螺旋CT的在寰枢椎病变中的应用价值。方法63例寰枢椎病变患者常规接受16层螺旋CT扫描,比较横断面以及M PR、SSD、M IP、VR重建等4种三维技术对病变的显示能力。结果寰枢椎旋转脱位6例,显示最佳的是VR及SSD;寰枢椎发育畸形、前倾和后仰脱位共17例,显示最佳的是M PR;寰枢椎骨折11例,显示最佳的是M PR;寰枢椎骨质破坏4例,显示最佳的是横断扫描与M PR;寰枢椎术后25例,显示最佳的是VR和M PR。结论对于寰枢椎病变来说,应该结合横断面和三维重建图像共同评价。三维重建中,M PR应用范围最广,显示效果也最好,但其它方式对于不同的病变也各有优势。

Abstract:Objective To evaluate the clinical usefulness of multi-slice spiral CT in diagnosis of atlantoaxial diseases.Methods 16 multi-slice spiral CT scan was performed on 63 cases of various atlantoaxial abnormalities.The ability of axial images,multipl-(anar) reconstruction(MPR),surface shaded display(SSD),maximum intensity projection(MIP),and volume rendering(VR)reconstruction for revealing these diseases was compared.Results Among the 63 cases,6 cases of atlantoaxial rotatory displacement were most intuitively displayed by VR and SSD;17 cases of atlantoaxial dislocation or malformation were most satisfactorily shown by MPR;11 cases of atlantoaxial fracture were most clearly displayed by MPR too;four cases of bone destruction were most clearly displayed by routine axial images and MPR;25 post-operation cases were most satisfactorily shown by VR and MPR.Conclusion Atlantoaxial diseases should be evaluated in combination with routine axial slices and different types of three-dimensional construction.Though MPR is the most efficient one of those four three-dimensional construction techniques,others have their own respective advantages.
