康复治疗对脑卒中吞咽障碍患者吞咽功能的影响

目的探讨早期介入持续2个月康复治疗对脑卒中吞咽障碍患者吞咽功能的影响。方法将60例脑卒中吞咽障碍患者随机分为治疗组(30例)和对照组(30例)。根据日本学者才藤分级法,将吞咽障碍分级评定。分别于发病7天内,开始进行2个月的以吞咽康复训练为主的综合治疗,对两组患者在康复训练前,2、4周,2个月后进行评价,并进行组内和组间比较。结果治疗前两组分级评分无显著差异(P>0.05)。治疗2、4周,2个月后均有显著差异(P<0.01)。治疗组显效率及有效率4周内增高最明显,2个月后仍有增高。结论治疗组脑卒中患者吞咽功能恢复,2周内效果明显,4周后进入缓升期,2个月后治疗组较对照组吞咽功能仍有恢复,故吞咽康复治疗应持续一段时间。

The effect of rehabilitation therapy ion function of swallowing in apoplectic patients suffering from dysphagia

Objective To assess the effect of rehabilitation therapy for two months on function of swallowing in apoplectic patients suffering from dysphagia.Methods Sixty apoplectic patients with dysphagia were randomized into two groups(the therapeutic group,control group).Evaluation of dysphagia was done according to the grades proposed by the Japanese author.Swallowing rehabilitation was the main component of comprehensive treatment,the treatment was maintained for 8 weeks from the beginning of the sickness.The evaluation was done at the beginning,2 weeks,4 weeks and 2 months of rehabilitation.The comparison was done between the two groups.Results There was no difference between the two groups at the beginning of treatment(P>(0.05).)Significant difference was found between the two groups after 2 weeks,4 weeks and 2 months of treatment(P<(0.01),) The effectiveness of the therapeutic group was significant at two weeks and increased in two months.Conclusion The effectiveness of early swallowing rehabilitation was significant at two weeks and increased slowly in 4 weeks.The recovery of swallowing furction in therapeutic group was better than that in control group in two months,so the swallowing rehabilitation should be maintained for a period of time.
