婴儿心肌致密化不全的超声心动图诊断

摘    要:目的 了解婴儿心肌致密化不全(又称海绵样心肌)的超声心动图诊断。方法 回顾分析2例婴儿海绵样心肌的超声心动图和临床表现。结果 婴儿海绵样心肌具有严重的心腔增大、室壁增厚、心肌呈网状窦隙并与室腔相通及内膜回声极不规则的声像图特征。心脏收缩功能显著减低,可合并心内膜弹力纤维增生或其他心血管畸形。表现呈进行性的充血性心衰。结论 婴儿海绵样心肌罕见,超声心动图是诊断该病的重要方法。

Abstract:Objective To study the echocardiography of myocardial noncompaction(spongy myocardium)in infants.Methods The manifestations of clinic and echocardionraphy of spongy myocardium in two cases of infants are studied retrospectively.Results Echocardiography of spongy myocardium in two patients revealed severe dilated cardiomyopathy affecting both ventricles with abnormally hypertrophied and irregular myocardial walls with a network of trabeculions and intertrabecular spaces.The systolic ventricular functions were severe reduced in the infants with spongy myocardium. The patients were complicated endocardial fibroleastosis or congenital cardiovascular malformation.The clinical courses of the disease were similar to congestive and dilatative cardiomyopathy.Conclusions spongy myocardium is a rare congenital anomaly. Echocardiographiy is important method to detected spongy myocardium in infant.
