低分子量肝素治疗急性脑梗死疗效的研究

目的 探讨低分子量肝素(LMWH)对急性脑梗死抗凝干预治疗的临床疗效。方法 将48例急性脑梗死患者分为低分子量肝素治疗组和对照组(常规治疗),每组24例,分别给予治疗前后神经功能缺损评分,检测治疗前后凝血指标和血液流变学指标变化。 结果 治疗组总有效率(83.3％)明显优于对照组(58.3％),神经功能缺损评分改善显著(P<0.01);治疗组凝血指标凝血酶原时间、部分凝血活酶活性时间与治疗前比较差异无显著意义(P>0.05),而纤维蛋白原降低(P<0.05),血液流变学变化较对照组明显改善。 结论 低分子量肝素治疗急性脑梗死,有助于神经功能的改善,抗凝作用强,其方法简便、安伞。

Clinical study on the treatment of cerebral infarction with low molecular weight heparin

Objective To study the therapeutic effect of low molecular weight heparin (LMWH) in anticoagulation treatment of cerebral infarction. Methods Forty- eight patients with cerebral infarction (PCI) were randomly divided into LMWH group ( n =24) and control group ( n =24). The neurological defect scoring system was used before and after treatment to evaluate the patients responses to LMWH. The blood coagulation and hemorheology parameters were also evaluated. Results In LMWH group the therapeutic effect (83. 3% ) was significantly greater than that of control group (58. 3% ). In LMWH group the neurological defect scoring and hemorheology parameters were also significantly improved ( P < 0. 01). There was no significant difference in pro-thrombin time and activated partial thromboplastin time between the two groups ( P > 0. 05). The concentration of fibrinogen was significantly lower in the LMWH group ( P < 0. 05). Conclusion Low molecule weight heparin has strong anticoagulation effect and can reduce neurological deficit of acute cerebral infarction. Its clinical application is effective, safe and convenient.
