小梁切除术后滤过不良的激光治疗

目的探讨激光治疗小梁切除术后滤过不良的方法及疗效。方法激光治疗小梁切除术后眼压控制不良87例（98只眼），其中行色素上皮残留氩激光切除术4只眼；经内路氩激光内口阻塞重新开放术17只眼，经内路YAG内口阻塞重新开放术11只眼，氩激光断线术59只眼，氩激光断线术联合经内路激光内口阻塞开放术7只眼。结果治疗前眼压4.56±0.76kPa，随诊16.2±7.3个月后眼压2.33±0.60kPa，治疗前后眼压有显著差异，P＜0.001。眼压正常89只眼（成功率90.8％）。主要并发症暂时性高眼压反应17只眼，低眼压、浅前房4只眼，虹膜炎15只眼，结膜穿孔1只眼。结论激光治疗小梁切除术后眼压控制不良是安全有效的。

Laser treatment for the poor infiltration after trabeculectomy in glaucoma

Objective：To evaluate the methods and efficiency of lasertreatment for poor filtration after trabeculectomy in glaucoma．Methods：Laser treatment was used in 87 glaucoma patients （98 eyes） whose IOP could not be controlled successfully after trabeculectomy．Of these，residual pigmented layer of iris in 4 eyes were excised by Ar laser；the obstructed inner apertures were reopened by Ar laser in 17 eyes，and in 11 eyes by YAG laser；Ar laser suture lysis by Ar laser in 59 eyes and Ar laser suture lysis combined with reopening of the inner aperture with YAG laser in 7 eyes．Results：In 89 eyes the IOP was normal，success rate 90．89％．Before laser treatment，the IOP was 4．56±0．76 kPa，and was 2．33±0．60kPa after laser treatment（P＜0．001，follow-up 16．2±7．3 months）．The main complications were transcient increased IOP（17eyes），ocular hypotension and flat anterior chamber（4 eyes），iritis（15eyes） and penetration of conjunctiva（1 eye）．Conclusion：Laser treatment for poor infiltration after trabeculectomy in glaucoma is safe and effective．
