规则肝切除治疗复杂肝内胆管结石临床分析

目的探讨复杂肝内胆管结石的临床特点及规则肝切除治疗复杂肝内胆管结石的临床疗效。方法回顾性分析2008年1月至2012年12月安岳县人民医院治疗的82例规则肝切除复杂肝内胆管结石患者的临床资料,分析患者肝内胆管结石的临床特征,规则肝切除的范围以及治疗效果。结果 82例肝内胆管结石患者均顺利完成规则肝切除手术,手术时间为(64.0±28.4)min,术中出血量为(84±26)mL。术后无结石残留病例,无术中及术后死亡病例,术后并发胆漏2例,并发术后创面渗血1例,均保守治疗治愈。术后规律随访1⁵年,结石复发率为10.9%。结论规则肝切除治疗复杂肝内胆管结石具有较好的临床效果,术后结石复发率低,具有较好的远期疗效。

Clinical Analysis on Regular Hepatectomy in the Treatment for Complicated Intrahepatic Bile Duct Stone

Objective To investigate the clinical characteristics of complicated intrahepatic bile duct stone and curative effect of regular hepatectomy in the treatment for complicated intrahepatic bile duct stone.Methods Clinical data of 82 cases of patients underwent regular hepatectomy for treating complicated intrahepatic bile duct stone from Jan.2008 to Dec.2012 in Anyue People＇s Hospital were retrospectively analyzed,and clinical characteristics,extent of regular hepatectomy and curative effect were analyzed.Results All of the 82 patients favourably completed regular hepatectomy,operative time was （64.0 ± 28.4）min,volume of hemorrhage was （84 ± 26） mL.There was not a single case of residue stone and death cases during operation and post-operation,there were 2 cases of post-operative bile leakage,and 1 cases of wound surface capillary hemorrhage,which were cured by conservative treatment.All the patients were given regular followup visits within the following 1 to 5 years,recurrence rate of intrahepatic bile duct stone was 10.9％.Conclusion Regular hepatectomy has good clinical effect on treating complicated intrahepatic bile duct stone,with low recurrence rate and ideal long-term curative effect.
