前房型人工晶状体植入术的临床效果观察

目的：观察PMMA弹性开放襻一体式前房型人工晶状体（AC－IOL）植入术的临床效果及并发症，评价其安全性.方法：回顾调查16例（AC－IOL）植入术后的视力、角膜、前房角及眼压情况，随访期1－18月，平均10．3月。手术包括超声乳化吸出AC－IOL植入术和Ⅱ期AC－IOL植入术。结果：术后1周视力＞0．5者占18．75％，0．1－0．5者占68．75％，＜0．1者占12．50％。角膜水肿发生率I期植入者为85．7％，Ⅱ期植入者为44．4％。虹膜周边前粘连发生率为68．8％（发生在襻支点周围者占其中的81．8％）。术后高眼压发生率为18．8％。结论：弹性开放襻一体式AC－IOL植入术手术简单，术后视力恢复良，不失为常规后房型人工晶状体植入失败的补救措施，但远期效果及并发症有待进一步观察和避免。

Clinical effects of the AC-IOL implantation

Objective To observe the clinical effects and complications of flexible open-loop anterior chamber intraocular lens and evaluate its safety. Methods Sixteen AC-IOL implanted eyes were followed up postoper-atively for 1 to 18 months, their vision, corneal and intraocular pressure were observed.The operation was phacoemulsification combined with primary or secondary AC - IOL implantation. Results One week postopera-tively,the visual acuity of 18.8% eyey was >0.5, 68.8% was 0.1 ~ 0.5 and 12.5% was < 0.1. There were corneal endothelial edema in 85.7% eyes with primary implantation and 44.4% eyes with secondary implantation. There were 68.8 % eyes with peripheral anterior synechia. There were 18.8% eyes with increased intraocular pressure. Conclusion The flexible open-loop anterior chamber intraocular lens implantation is easy to do. We can chose this method when without the posterior capsular support,but the long term effects and complications should be observed.
