举宫杯腹腔镜全子宫切除术的临床研究

摘    要:目的:探讨用举宫杯行腹腔镜全子宫切除术的临床效果。方法:收集2010-03-2012-10我院收治有手术指佂的子宫肌瘤、子宫肌腺症、功能性子宫出血、CIN3患者100例(实验组50例,对照组50例)。实验组采用举宫杯行腹腔镜全子宫切除术,对照组行腹腔镜辅助阴式全子宫切除术。比较两种术式的手术时间、出血量及住院时间、并发症等情况。结果:实验组术中出血量为(50±12)ml。术后肛门排气时间为(18.0±2.0)h。术后住院时间为5d,均优于对照组(P〈0.05)。结论:举宫杯腹腔镜全子宫切除术具有创伤小、术后并发症少、恢复快及住院时间短等优点,优于腹腔镜辅助阴式全子宫切除术,值得临床推广应用。

Total iaparoscopic hysterectomy with cup--type uterine manipulator

Abstract:Objective:To investigate the clinical effects of total laparoscopic hysterectomy with cup--type uterine manipulator. Methods: A total of 100 patients with hysteromyoma, uterine adenomyosis, dysfunctional uterine bleeding and CIN3 hospitalized in our hosipital from March 2010 to October 2012 were selected (50 cases in experimental group and 50 cases in control group). Total laparoscopic hysterectomy with cup- type uterine manipulator was performed in experimental group while laparoseopically assisted vaginal hysterectomy was performed in control group. Operation time, amount of bleeding in operation, length of hospital stay and complications of the two groups were compared. Results: The amount of bleeding in operation (50±12) ml,time of passage of gas by anus after operation (18.04±2) h and length of hospital stay after operation (Sd) were less in exprimental group than those in control group (P〈0.05). Conclusion: Total laparoscopic hysterectomy with cup--type uterine manipulator shows advantages of small trauma, few postoperative complication, rapid recovery, and short length of stay. It issuperior to laparoseopic assisted vaginal hysterectomy, and worthy of clinical application.
