摘    要:目的探讨2种内固定术治疗胫骨干中下1/3骨折的疗效。方法 84例胫骨干中下段1/3骨折患者分为观察组与对照组各42例。观察组行骨折端微创切口经皮锁定钢板内固定术,对照组行完全切开复位锁定钢板内固定术,记录2组手术时间、术中出血量、骨折愈合时间、住院时间,并进行疗效比较。结果观察组住院时间、手术时间((14.26±2.67)d、(1.43±0.16)h)与对照组((14.63±2.32)d、(1.45±0.11)h)比较差异无统计学意义(P〉0.05),观察组骨折愈合时间((5.52±0.28)个月)、术中出血量((120.0±46.2)mL)与对照组((9.32±0.32)个月、(155.0±50.2)mL))比较差异有统计学意义(P〈0.05);观察组治疗优良率(92.9%)高于对照组(83.3%)(P〈0.05)。结论骨折端微创切口经皮锁定钢板治疗胫骨干中下1/3骨折效果优于完全切开复位锁定钢板内固定术。

Abstract:Objective To observe the effect of two internal fixation methods on the distal tibial shaft fractures.Methods Eighty-four patients were randomly divided into two groups:observation group(n=42)receiving minimally invasive percutaneous locking plate at the fracture end,and control group(n=42)receiving completely open reduction plus internal fixation with locking plate.The operation lasting time,intraoperative blood loss volume,time of fracture healing and hospitalization stay were recorded and compared between two groups.Results There were no significant differences in the operation lasting time and hospitalization stay between observation group((14.26±2.67)days,(1.43±0.16)hours)and control group((14.63±2.32)days,(1.45±0.11)hours)(P〈0.05).The fracture healing time and blood loss volume were(5.52±0.28)months and(120.0±46.2)mL in observation group,significantly different from those in control group((9.32±0.32)months,(155.0±50.2)mL)(P〈0.05).The excellent and good rate was 92.9%in observation group,significantly higher than that in control group(83.3%)(P〈0.05).Conclusion Minimally invasive percutaneous locking plate at the fracture end is superior to completely open reduction plus internal fixation with locking plate for distal tibial shaft fracture.
