体外循环后的严重支气管痉挛

【目的】通过2例少见的体外循环后严重支气管痉挛的患者探讨其发生的可能原因和机理。【方法】接受冠状动脉架桥手术的两例患者，当手术结束恢复机械通气体外循环准备减流量时，发生了严重的支气管痉挛。患者过去史中都没有哮喘体质。【结果】体外循环结束前进行积极的多种支气管扩张剂治疗。患者顺利脱离体外循环，术后无症状。【结论】体外循环后严重的支气管痉挛十分少见。在排除其它类型通气衰竭才能予以诊断。需要应用支气管扩张剂进行治疗。

Severe bronchospasm during cardiopulmonary bypass

Objective] To describe the rare problem of severe bronchospasm occurring during cardiopulmonary bypass in two patients who received CABG operatious. Methods] Severe bronchospasm became apparent on attempting to resume controlled ventilation prior to weaning from cardiopulmonary bypass. Results] Aggressive therapy with multiple bronchodilating agents was necessary before cardiopulmonary bypass could be discontinued. The bronchospasm resolved after surgery and patients were discharged uneventful. Conclusions] Severe bronchospasm during cardiopulmonary bypass is rare. It should only be diagnosed after ruling out other reasons for failure to ventilate. Treatment with intravenous bronchodilators is required. The cause is unknown.
