不典型肝脓肿16例低场MRI征象分析

目的:分析不典型肝脓肿低场MRI征象的临床诊断价值.方法:回顾性分析经手术病理或穿刺引流证实的16例23个肝脓肿,分析不典型肝脓肿低场MRI及增强影像表现.结果:发现纤维肉芽性脓肿13个(56.5%),增强后延迟强化;实质性团块状脓肿5个,均伴有完整包膜且增强后有强化表现;小房腔型脓肿(≤2.1cm)5个,增强扫描才能清晰显示脓腔壁,呈延迟强化.结论:肝脓肿病理演变是造成肝脓肿MRI征象不典型的主要原因,低场MRI可有效提高肝脓肿的临床诊断正确率.

Study on 16 Cases of Atypical Hepatic Abscess on Low Field of MRI

Objective: To investigate the features of atypical hepatic abscess on low field of MRI.Method:MRI findings in 16 cases of atypical hepatic abscess patients which were proved by surgical pathology or puncture drainage were retrospectively analyzed.Result:13 lesions(56.5%) were composed of granulomatous and fibrous tissue,It showed obviously delayed enhancement.lesions appeared circumscribed with the complete capsule and showed rim enhancement after contrast.5 lesions(21.7%) appeared as a small cavity(≤2.1cm),and showed rim enhancement of wall only after contrast.Conclusion: Atypical MRI findings of liver abscesses result from individual differences of pathologic changes and regressions.And low field MRI can be improved the diagnostic accuracy.
