黄体期泌乳素水平变化与体外受精-胚胎移植结局的关系

目的:探讨黄体期泌乳素(PRL)水平变化与体外受精-胚胎移植(In Vitro Fertilizationembryo transfer,IVF-ET)结局的关系。方法:本研究收集了2010年1月至2010年6月就诊我中心228例进行IVF-ET患者移植后监测血清泌乳素,并对其体外受精-胚胎移植结局的关系进行分析。结果:228例行IVF-ET的患者在移植当日,血清PRL平均值681.6±253.4 uIU/mL,与基础血清PRL相比,明显升高,两者比较具显著统计学差异,P<0.01;228例患者中妊娠110例,未妊娠118例,总妊娠率48.2%;妊娠组与未妊娠组在移植0、2、4、6d血清PRL浓度比较P>0.05;在第8天两者比较具显著统计学差异,P<0.01。控制良好组妊娠率53.1%,比控制过度组、欠佳组的妊娠率明显增高,具显著统计学差异,P<0.01。结论:黄体期血清PRL水平变化与IVF-ET结局有一定的关系,黄体期血PRL浓度较高时用溴隐亭控制在一定浓度时有利于提高妊娠率。

The Relationship Between the Change of Prolactin Levels and the Outcome of IVF-ET during Luteal Phase

Objective: To investigate the relationship of prolactin levels and the outcome of in vitro fertilization-embryo transfer(IVF-ET) during the luteal phase.Method: The study collected 228 cases who received IVF-ET transplant in our treatment center during Jan.2010 to Jun.2010,all the patients were divided into four groups according to the result of the monitoring of serum prolactin,And analyzed the relationship with outcome of IVF-ET.Result: The mean serum PRL level of the 228 patients who received routine IVF-ET transplantation was 681.6 ± 253.4 uIU / mL,which was significantly higher compared with the basic serum PRL(P <0.01);Among the 228 patients,110 cases were pregnancy,118 cases were non-pregnant,the total pregnancy rate was 48.2%;The mean serum PRL of the pregnancy group was rising after transplantation,which was significant on the 8th day of the transplantation,while the non-pregnancy group was without the trend,and its average level decreased on the 8th day.The serum PRL concentrations of the pregnant group and the non-pregnant group had significant difference on the 0,2ed,4th,6th days,P> 0.05,which had no significant difference on the eighth day,P <0.01.Pregnancy rate of the group was 62.8%,which was significantly higher than that of the other group,the pregnancy rate of D group was 26.67%.The pregnancy rate of the good control group was 53.1%,which was better than that of the over control group and the poor control group,P<0.01,which was statistically significant.Conclusion: The luteal phase serum PRL levels have a certain relationship with IVF-ET outcome,bromocriptine can be used while the luteal phase serum PRL level is of higher concentration,which can help to improve the pregnancy rate.
