甘精胰岛素联合瑞格列奈在新诊断2型糖尿病中的应用研究

目的 探讨甘精胰岛素联合瑞格列奈治疗新诊断的2型糖尿病的临床疗效,以及对胰岛β细胞功能的影响.方法 选取80例新诊断的2型糖尿病患者,随机分为观察组和对照组各40例,观察组给予瑞格列奈片及甘精胰岛素,对照组给予格列吡嗪片及甘精胰岛素,观察两组患者空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA1C),以及应用胰岛素第1时相分泌和稳态模型评价胰岛β细胞功能.结果 观察组患者总有效率95.00%,明显高于对照组的80.00%,二者差异具有统计学意义(P<0.05);治疗6个月后,两组患者FPG、2hPG及HbA1C水平均下降,但观察组下降更明显(P<0.05),并且观察组患者FINS、空腹C肽、AIR 和HOMA-βF优于对照组(P<0.05).观察组患者低血糖发生率为7.50%,低于对照组的30.00%,差异具有统计学意义(P<0.05).结论 甘精胰岛素联合瑞格列奈能有效控制新诊断2型糖尿病患者的血糖,保持和恢复胰岛β细胞功能,低血糖发生率低,安全可靠,值得临床推广应用.

Research on Repaglinide in Insulin Glargine Combined with Newly Diagnosed Type 2 Diabetes

Objective: To investigate the effect of insulin glargine combined with repaglinide in the treatment of newly diagnosed type 2 diabetes,as well as on the function of islet β cell effect.Method: 80 patients with newly diagnosed type 2 diabetic patients,were randomly divided into the observation group and the control group,40 cases in each group,the groups were observed given repaglinide tablet and insulin glargine,the control group was given glipizide and insulin glargine,two groups were observed in patients with impaired fasting glucose(FPG),2h postprandial plasma glucose(2hPG),glycosylated hemoglobin(HbA1C),as well as the application of first phase insulin secretion and homeostasis model assessment of islet B cell function.Result: In the observation group,the total effective rate was 95%,It was significantly higher than that in control group 80%,the difference was statistically significant(P < 0.05);after 6 months of treatment,two groups of patients with FPG,2hPG and HbA1C water decreased,but the observation group decreased more obviously(P < 0.05),and in the observation group,FINS fasting C peptide,AIR and HOMA beta F is better than that of control group(P < 0.05).In the observation group,hypoglycemia occurred at a rate of 7.50%,lower than 30% in the control group,the difference was statistically significant(P < 0.05).Conclusion: Insulin glargine plus repaglinide can effectively control in newly diagnosed type 2 diabetic patients blood glucose,maintain and restore the function of islet β cell,hypoglycemia incidence is low,safe and reliable,worthy of clinical application.
