胰岛素泵在肥胖2型糖尿病患者中的临床应用

摘    要:目的:比较持续皮下胰岛素注射(CSII)和多次皮下胰岛素注射(MDI)在肥胖2型糖尿病(T2DM)患者中的应用。方法:CSII组206例,MDI组180例,均进行短期胰岛素强化治疗,比较血糖水平、血糖达标时间、达标时胰岛素用量、低血糖发生率等指标。结果:CSII组较MDI组血糖达标时间更短(P<0.05),且胰岛素用量较低(P<0.05),血糖漂移幅度较小(P<0.05),两组低血糖的发生无显著性差异。结论:与MDI组比较,CSII能在单位胰岛素用量更少的情况下,更快、更平稳地控制血糖,从而降低了体质量的增加和血管并发症的风险。

Abstract:Objective: To compare the effects of two different ways of insulin delivery in the treatment of type 2 diabetic patients with obesity(T2DM).Methods: We conducted continuous subcutaneous insulin infusion(CSII) for 206 and multiple daily insulin injection(MDI) for 180 type 2 diabetic patients with obesity and compared the 2 groups in the blood glucose(BG) level,time needed to achieve good BG control,insulin dose and incidence of hypoglycemic episodes. Results: Compared with MDI,CSII significantly reduced the time for achieving the target BG,achieved better glucose control and lowered the dose of insulin(P<0.05),with no difference in the incidence of hypoglycemia between the 2 groups(P>0.05). Conclusion: CSII appeared to be superior to MDI in controlling BG,with lower insulin dose,less time,better results and fewer complications for type 2 diabetic patients with obesity.
