立体定向微创与常规开颅手术治疗高血压脑出血的疗效比较

目的比较立体定向微创术与常规开颅手术对高血压脑出血的临床疗效。方法研究对象选取2013年3月~2014年3月在我院神经外科诊断治疗的77例高血压脑出血患者,随机分为常规开颅组和立体定向组,比较两组患者手术时间、术中出血量、术后神经功能缺损评分、及并发症。结果立体定向组患者手术时间(48.53±10.96)min,较常规开颅组(73.86±20.83)min明显缩短、术中出血量(26.75±9.36)ml较常规开颅组(147.39±30.42)ml显著减少,差异有统计学意义(P0.05或P0.01);术后神经功能缺损评分及并发症较对照组有显著性差异,比较有统计学意义(P0.05或P0.01)。结论立体定向微创术治疗高血压脑出血创伤小、疗效好、安全性高,可在临床推广应用。

Comparison of clinical effects of minimally invasive stereotactic operation and craniotomy for hypertensive cerebral hemorrhage

Objective To compared the clinical effect of minimally invasive stereotactic operation and craniotomy for hypertensive cerebral hemorrhage.Methods Totally 77 patients of hypertensive cerebral hemorrhage from Mar 2013 to Mar 2014 in neurology department of our hospital were selected to randomly assign into routine craniotomy group and stereotactic group.Compared the two groups operation time,intraoperative hemorrhage,NIHSS and complications.Results The operation time of stereotactic group was（48.53±10.96）min,the routine craniotomy group was（73.86±10.83）min,the intraoperative hemorrhage of stereotactic group was（26.75±9.36）ml,the routine craniotomy group was（147.39±30.42）ml,there were statistically significant difference（P〈0.05 or P〈0.01）;The score of NIHSS and complications in stereotactic group was also significant differences in the routine craniotomy group（P〈0.05 or P〈0.01）.Conclusion Minimally invasive stereotactic operation has the characteristics of small injury,good efficacy,and high safety in the treatment of hypertensive cerebral hemorrhage,it can be extensively applied in clinic.
